Does someone need to be declared incompetent to have a representative payee for social security?

my fathers wife states a doctor through Social Security had him declared incompetent. Can this happen without him going to court? If so how do and can I get this reversed? My father at 61 may at times be a little forgetful but by no means is he incompetent.
